Compliance and Eligibility
The Millsaps College athletic department and all student-athletes must adhere to all compliance and eligibility guidelines of the College, the National Collegiate Athletic Association (NCAA), and the Southern Athletic Association (SAA).
STUDENT-ATHLETE FORMS
The following forms must be filled out in its entirety and submitted to your respective coach and then the athletic department prior to a student-athlete being cleared to participate in a sport. A student-athlete will not be allowed to participate unless the forms are completely filled out.

Please fill out one of the Sports Information Forms below. If you are a returning student-athlete at Millsaps please fill out the appropriate form. If you are a new student-athlete at Millsaps (incoming freshman, transfer, or first year on a team at Millsaps) then use the form for new student-athletes. By filling out and returning this form, you are giving permission to the Millsaps Athletic Communications Office to use this information to publicize Millsaps Athletics and complete award nominations. Please be as complete and specific as possible when filling out the form. MEDICAL CLEARANCE
NCAA student-athletes are required to complete a sports physical prior to participation in athletics. All physicals for this academic year must be completed after June 1st. All physicals must be signed by a Medical Doctor or Doctor of Orthopeadics. Physicals completed by any other health care provider must be signed by their supervising physician. Physicals performed by a chiropractor, not signed by a physician or on a form other than the Millsaps College intercollegiate athletic form will not be accepted. A new physical will be required at the cost of the student-athlete. Please use the physical form found on the Millsaps sports medicine web page (http://gomajors.com/index.aspx). Physicals will be provided on campus upon arrival at Millsaps and prior to participation in athletics. Physical dates will be assigned to teams in the fall of 2019. If you cannot attend the scheduled fall dates you are responsible for obtaining your own appointment for a sports physical.
Insurance
Millsaps College and the NCAA requires that all student-athletes show proof of insurance (i.e. under a parent, employer, etc...) prior to participating in an intercollegiate athletic program.
Millsaps College student-athletes are also covered under the athletic department's policy as a secondary medical insurance policy. It has a deductible of $5,000.00. The secondary plan will not pay until after the primary plan has paid its benefits if any. Any amount paid by the primary plan for an eligible expense under the secondary plan may satisfy all or a portion of the deductible
Any student-athlete wishing to file a claim under the athletic department's secondary medical insurance policy must contact the athletic training office to ensure a claim is filed and receive any required information. Please see the informational letter below for further information.

NCAA COMPLIANCE
In addition student-athletes must fill out all NCAA compliance forms prior to playing their first contest. The forms are listed below and must be printed and filled out prior to the compliance meetings. Those meetings will be led by Millsaps College's compliance coordinator.
If you are currently enrolled at a four-year NCAA institution and wish to transfer to Millsaps College, the athletic department must receive written permission to contact from the athletic director or compliance coordinator at your current institution. If you wish you may use a SELF-RELEASE FORM.

ADMISSIONS AND ACADEMIC STANDARDS
In order to compete in intercollegiate athletics, Millsaps College student-athletes must meet all NCAA Division III and SAA eligibility rules.
All student-athletes must be in good academic good standing as defined by Millsaps College in order to compete in intercollegiate athletics. These standards consist of the following:
- The Student Athlete must remain enrolled in 12 credit hours per semester
- The Student Athlete must be making satisfactory academic progress.
- The Student-Athlete must be in good academic standing.
ALCOHOL, TOBACCO AND DRUG USE
The Millsaps College athletic department supports the understanding that alcohol, tobacco and other drug use is detrimental to the student and can be a violation of team, department and college rules. Specifically: ALCOHOL consumption by a student-athlete under 21 years of age is illegal. The use of DRUGS (recreational or performance enhancing) is illegal. The use of any tobacco products at an NCAA sponsored event is a violation and will affect eligibility. Any of these violations will warrant disciplinary action from the department and/or college.
As per rule 11.1.5 in the NCAA Division III manual, the use of tobacco is prohibited by all game personnel (e.g. coaches, trainers, managers, and game officials) in all sports during practice and competition.

SPORTS WAGERING
The following individuals shall not knowingly participate in sports wagering activities or provide information to individuals involved in or associated with any type of sports wagering activities concerning intercollegiate, amateur, or professional athletics competition:
(a) Staff members of an institution's athletic department;
(b) Non-athletics department staff members who have responsibilities within or over the athletics department (e.g. chancellor or president, faculty athletics representative, individual to whom athletics reports);
(c) Staff members of a conference office; and
(d) Student-athletes
NCAA DIVISION III PHILOSOPHY STATEMENT
Colleges and universities in Division III place highest priority on the overall quality of the educational experience and on the successful completion of all students' academic programs. They seek to establish and maintain an environment in which a student-athlete's athletics activities are conducted as an integral part of the student-athlete's educational experience, and in which coaches play a significant role as educators. They also seek to establish and maintain an environment that values cultural diversity and gender equity among their student-athletes and athletics staff.
